The following must be on file in the Athletic Office for students to be eligible to participate in athletic activities:
1) Sports Qualifying Physical Form
2) MSHSL Form
3) Insurance Form
4) $50.00 Fee Paid
5) Attend the Annual All Sports Eligibility Meeting
Sports Qualifying Physical Form
A sports physical is required every three years. Most athletes will get a physical in the 7th grade and again in the 10th grade.
The Minnesota State High School League Eligibility Form must be signed by both the parent and student. This form needs to be completed once per school year. If you have signed this form for a fall sport, you do NOT need to complete this form again if you participate in a winter or spring sport.
St. Charles High School offers student accident insurance thru Student Assurance Services. Pamphlets with detailed information are available at all registration meetings as well as in the Athletic Office. If you have adequate insurance and are not interested in purchasing additional coverage, a waiver form must be signed. This form only needs to be completed once per school year.

A $50.00 fee is required for each participant/sport. There is a $120.00 max per family.
ANNUAL ALL SPORTS ELIGIBILITY MEETING
Students and parents are required to attend the Athletic Eligibility Meeting each year. The meeting is scheduled for Tuesday, August 20 at 7:00 p.m. at the Elementary Auditorium.
