IMMUNIZATION REQUIREMENTS


Minnesota Law requires all children enrolling in elementary or secondary school to show evidence of immunization or properly document exemptions.
The St. Charles School District requires a print out from your health care provider as proof of immunizations. No parent phone calls or handwritten notes can be accepted.
If your child will be entering School Readiness, Kindergarten or 7th grade in the Fall, please turn in the proper documentation of immunizations as soon as possible. Letters will be sent out in late August to those families whose students are not in compliance.  Students attending our after-school care programs or wrap-around care programs must also be compliant with immunizations per state law and get immunization records to the district prior to enrollment.    
Students who cannot be vaccinated due to a special medical condition or whose parent/guardian is conscientiously opposed to immunization may receive a legal exemption through:
• A statement signed by a physician that the immunization is contraindicated for medical reasons, or confirmation that adequate immunity exists.
• A notarized statement signed by the student's parent or legal guardian stating that the prescribed immunization(s) are contrary to conscientiously held beliefs of the parent or guardian.
